Patch updates articles/problem-reports/article.sgml revision 1.55 as follows:

1. Clarifies that synopses with [patch] and [maintainer update] should include the brackets

2. Adds hyperlink to web-based PR submission form

Fix: Apply included patch to articles/problem-reports/article.sgml revision 1.55 <http://www.freebsd.org/cgi/cvsweb.cgi/~checkout~/doc/en_US.ISO8859-1/articles/problem-reports/article.sgml?rev=1.55;content-type=text%2Fplain>
