Summary: Installed 4.1 on an Compaq 1500. As noted in errata, boot hangs. How do I create a boot disk other than the install disks?

I think I successfully installed 4.1 on a Compaq 1500 server.  Per the errata there is a bug in the boot0.  The solution is to boot using floppies.  My question is how do I create a boot floppy?
