For anyone unwilling to take risks of unpacking the archive, I already did that in a safe environment.  The archive contains three pictures that can be summarized as the kernel failed to mount the root filesystem from the installation USB media.
The error is CAM status 0x50.  Apparently it comes from umass.c:
ccb->ccb_h.status = CAM_REQ_CMP_ERR | CAM_DEV_QFRZN;

I'm sorry that this PR never got looked at, but by now, 12.0 is long out of support.
