In the attached screenshot: <https://reviews.freebsd.org/D37235#inline-228496>, apparently unsubmitted. From conversation in IRC with the author (pauamma) of this inline comment, I _do_ believe that the 'Submit' button was used to add the comment; to make it visible and available for response. 

Elsewhere in the same review: <https://reviews.freebsd.org/D37235#inline-228542> from gjb, apparently unsubmitted, with my submitted response …

… and so on.

This is unfortunately how phabricator is designed, it not perfect but we cannot do very much on our side.  The best way to solve this issue is reporting to upstream and collaborating with them to improve this.  Please note that Phabricator is EoL'd but there is a community fork at https://we.phorge.it/
