tac_plus port update

  - fixed wrong config file name in tac_plus.sh
  - added optional patch for change the text displayed
    when users are prompted to enter a username

NOTE:

files/patch-ad and files/tac_plus.conf.example added
files/tacacs.conf.example removed

The update is done with some changes including: 

- Change the name of the optional patch to patch-ba so that there will 
be less chance that we should rename this file when adding new 
non-optional patch file. 
- Add pre-clean target to remove the optional patch from the patches directory. 
- Added back $Id$ line.
