	First of all, this patch is approved by Addison Wesley and Ted Mittelstaedt
	
	The first patch contains fixes, the second one is whitespace only

	These patches include:
	- Copyright and ISBN enhancements for the japanese version of the book
	- Stuff mentioned on the errata page of the book
	- Many whitespace fixes
	- Some additional changes requested by Ted Mittelstaedt

I've Committed  your changes, but we need to notice 
that some URL links are broken (ie no file found, no 
directory found, ...) 

I've not be able to find the replacing URL (for example, in ftp.novell.com, there is no file named tcp16.exe...)
