- Add missing recently added foreign language subdirs:
  - doc/el_* (Greek)
  - doc/no_* (Norwegian)
  - doc/pl_* (Polish)
  - doc/pt_* (Portugese)
- Actually add non-English language doc/ subdirs to the sample refuse file
  (previously listed only in refuse.README); it does not hurt anyone NOT
  cvsupping the doc tree
- Remove now-useless doc/<two-letter code> subdirs as they have been 
  removed/renamed in the tree some time ago
