Notes in the SGML Primer PDF file are rendered incorrectly, running off
the page.  See page 12 of the PDF version of the SGML Primer.  In the Meaning
column for nsgmls error 5, there is a note "a".  This looks fine in HTML,
but the PDF version doesn't wrap and runs off the right side of the page.

This PR is way old. I will try to figure out what needs to be done and if i am capable enough i will also fix it. 
That is as long as the problem still occurs

I've just verified that it is still a problem.  The PDF still runs off
the page as described in the PR.  The stylesheets do not linewrap the
table notes.  They only support very short notes that fit on one page.
The easiest solution is to modify the content to work around this
problem.

If there are such parts in the documentation, those should be checked
