/usr/ports/mail/qmail/distinfo has the following info for outgoingip.patch
MD5 (outgoingip.patch) = 27e07ea6328cd763481c6d8887b20fe2
SIZE (outgoingip.patch) = 6841

The actual file is 6839 bytes and has a MD5 sum of 3c9277dcf5f9b4b6d3a270fb3abf7994

I pulled the file down from:
http://www.qmail.org/outgoingip.patch
http://qmail.geto.net/outgoingip.patch
http://qmail.agarik.com/outgoingip.patch

All three had the same contents, size and md5 sum.

Fix: Edit/patch /usr/ports/mail/qmail/distinfo
How-To-Repeat: cd /usr/ports/mail/qmail
make WITH_OUTGOINGIP_PATCH=yes
