The mt(1) manpage synopsis doesn't indicate that the command argument is
sometimes required.  Many similarly-trivial problems exist.

Fix: I've organized the commands by synopsis type, but left them
poorly organized within categories to limit diffs, but that was
fairly unsucessful.  Maybe they should be changed at this time too.

I also added some stuff, made a few things consistant, and cleaned up.
