- Update to 0.4.7
- Install documents and support NOPORTDOCS
- Use DEFAULT_PGSQL_VER and BROKEN_WITH_PGSQL, then PgSQL 8.1 users don't
  need to specify WANT_PGSQL_VER=81
- Use Makefile's install procedure
