Description Slawomir Wojciech Wojtczak 2010-02-18 10:10:01 UTC
After adding a 1TB SATA disk to this IBM x226 (attached at powered down)
I created some filesystems on it and mounted it, short time after that
we saw kernel panic and reboot with these error messages:

Feb 17 21:54:09 x226 kernel: ad2: timeout waiting to issue command
Feb 17 21:54:09 x226 kernel: ad2: error issuing FLUSHCACHE48 command
Feb 17 21:54:10 x226 kernel: ad2: TIMEOUT - READ_DMA48 retrying (1 retry left) LBA=1153433663
Feb 17 21:54:10 x226 kernel: ad2: FAILURE - READ_DMA48 status=51<READY,DSC,ERROR> error=10<NID_NOT_FOUND> LBA=1153433663
Feb 17 21:54:10 x226 kernel: GEOM_JOURNAL: Error while reading data from ad2s1 (error=5).
Feb 18 00:01:10 x226 kernel: ad2: TIMEOUT - READ_DMA retrying (1 retry left) LBA=72617663

.. and kernel panic next.

Below I attach dmesg/pciconf/fstab/loader.conf outputs/content:
