I've now seen this crash twice in a week

One of the disks on my isci(4) controller is going bad, and sometimes hangs and times out on the controller.

When this happens, and the controller goes to remove the device, it panics the system.

I wasn't able to use the debugger prompt because I had to reset the IPMI interface to get it to let me connect, so lost the USB keyboard.

I have replaced this hardware with an LSI controller.

This hardware is not current anymore.
