I have an external USB disk that works perfectly when connected to a running system, but becomes inaccessible (no /dev nodes) after rebooting while it is connected. Disconnecting and reconnecting the USB cable makes it work again. Here is the dmesg output, which ends in some USB error messages that are probably related.
