simplified test case
A simplified test case is attached and just to be executed as:
$ docbook2pdf test.sgml
I opened a thread in: https://lists.oasis-open.org/archives/docbook/201602/msg00021.html I have adde <gabor@FreeBSD.org>, the maintainer of textproc/docbook-utils, to the Cc: list (and I even suggest assign the issue to him/her) because while making this port textproc/docbook-utils, it spills out the same errors when the port wants to make the SGML written documentation; the current assigned port textproc/dsssl-docbook-modular only delivers files from world to our system; Reassign to textproc/docbook-utils maintainer.
